Inquiry Forms
Make the form couples fill out when they want to work with you, share its link, and add it to your website. Every inquiry arrives in your CRM as a new contact.
An inquiry form is the form couples fill out when they'd like to work with you. Put it on your website, in your Instagram bio, or in your email signature. When a couple sends it, Folia adds them to your CRM as a new contact with their answers already filled in, so you never have to copy details out of an email again.
This guide walks you through it from start to finish. You don't need any technical knowledge, and nothing is public until you choose to publish it.
Step 1: Create your form
- In the sidebar, under Administration, click Leads, then click Forms.
- Click Create my form. Once you have a form, this button is called New form, at the top right of the page.
Your new form opens right away. It already has the six questions couples are usually asked: their name, email, phone number, event date, estimated guest count, and a box to tell you about their day. You can keep them, change them, or add your own.
Before you create your first form, the page lists those questions and which detail on the contact each answer fills in. Click Preview the form to see them exactly as couples will.
Step 2: Make the questions your own
Open the Questions tab. Your questions are on the left, and a Preview of the form is on the right. The preview changes as you type, so you always see exactly what couples will see.
To change a question, click it. You can edit:
- Question: the words couples see, such as "When is your wedding?"
- Help text: an optional hint shown under the question, such as "A rough date is fine."
- Answer type: the kind of answer you want. See the table below.
- Saves to: which detail on the couple's contact the answer fills in. See Where the answers go.
- Required: turn this on if couples must answer before they can send the form. Required questions show a red star.
To add a question, click Add question at the bottom of the list.
To remove a question, click the trash can on its right.
To change the order, drag a question up or down by the dotted handle (⠿) on its left.
| Answer type | Good for |
|---|---|
| Short text | A single line, like a name or a venue |
| Paragraph | A longer answer, like "Tell us about your day" |
| An email address. Folia checks that it looks like one. | |
| Phone | A phone number |
| Number | A number, like a guest count or a budget |
| Date | A date, like the wedding day |
| Dropdown | One choice from a list you write |
| Checkboxes | Any number of choices from a list you write |
| Yes / No | A simple yes or no |
For a Dropdown or Checkboxes question, type the choices under Options.
When you're happy with your questions, click Save at the top right.
Where the answers go
Each question can fill in one detail on the couple's contact. In the starter form, for example, the "Event date" question fills in the contact's Event date, so the date is already there when you open the contact.
To choose, open the question and pick from its Saves to menu:
- Contact details: Full name, Email, Phone, Address, Event date, Location, Guest count, Budget, Source, Notes, and the name, email, and phone of an additional person, such as a partner.
- Custom fields: any extra fields you've added to your contacts.
- Don't save to the contact: for answers you only want to read. You'll still find them with the submission, on the form's Submissions tab.
Some details need a matching answer type. Guest count and Budget need a Number question, and Event date needs a Date question. Details that don't match are grayed out in the menu. Each detail can be filled in by only one question.
Step 3: Add your logo
Open the Design tab. Its choices are on the left, and the Preview on the right shows each one as you make it.
- Logo: click Upload and choose your logo from your computer. A PNG with a transparent background looks best. To use a different image, click Replace. To take the logo off, click Remove.
- Logo size: Small, Medium, or Large.
- Alignment: line up your logo, heading, and intro on the Left or in the Center. Questions always line up on the left.
Click Save when you're done. Your logo appears on the form's own page and on your website. It's left out of the contact form on your directory profile, which already shows your name.
Step 4: Choose the wording and where new contacts go
Open the Settings tab. Everything here is optional except the form name.
- Form name: only you and your team see this. If you make more than one form, use names that tell them apart, like "Website inquiry" and "Wedding fair".
- Heading and Intro: the title and the short welcome at the top of the form.
- Thank-you message: what couples see after they send the form.
- Redirect after submit: if you have a thank-you page on your own website, paste its address here. Couples go to that page instead of seeing the thank-you message.
- Stage: where new contacts start in your pipeline. First stage in your pipeline is usually New Inquiry.
- Owner: the person on your team that new contacts are assigned to.
- Source: a note about where inquiries from this form come from, like "Website". It's used when none of your questions saves to Source.
Click Save when you're done.
Step 5: Publish your form
Click Publish at the top right. If you have changes you haven't saved yet, the button says Save & publish and does both.
You'll see Form is live, and the form's status changes from Draft to Live. Its link now works.
If something needs fixing first, clicking Publish shows a short list called Before this can go live instead. Fix each item on the list, then click Publish again.
Step 6: Share the link
Every form has its own link. Couples who click it see your form on a page of its own.
- Click Share at the top right of your form.
- Under Link, click the copy button next to the link.
- Paste the link anywhere couples find you:
- your Instagram bio
- your email signature
- an "Inquire" button on your website
- a reply to a couple who asked about your services
To see your form the way couples do, click the button next to the copy button. It opens your form in a new tab.
You can also copy a live form's link from the forms list: click ⋯ next to the form, then Copy link.
Step 7: Put the form on your website
You can also show the form right on a page of your website, such as your Contact page, so couples never leave your site. It takes one copy and one paste.
- In Folia, open your form and click Share.
- Under Embed on your site, click Copy. This copies a short piece of code. You don't need to read it or change it.
- In your website builder, open the page where the form should go and add a code block. The table below shows what it's called in popular website builders.
- Click inside the new block and paste.
- Save or publish the page, then visit it to check that your form shows up.
| Website builder | What to add |
|---|---|
| Squarespace | A Code block |
| Wix | Embed Code, then Embed HTML |
| Showit | An Embed Code element |
| WordPress | A Custom HTML block |
| Webflow | A Code Embed element |
The code only shows your form. It doesn't change anything else on your website. The form fits the width of your page and grows as couples fill it in. On some website builders, such as Wix, the block stays the size you draw it, and a form taller than the block scrolls inside it. Drag the bottom of the block down until the whole form shows, Send button included. Then switch to the mobile view and do the same there, since the form is taller on a phone.
Step 8: See your inquiries
When a couple sends your form, Folia adds them to Contacts in your CRM, in the stage you chose in Settings, with their answers filled in.
- Folia lets you know. When a couple sends your form, your firm's owners get an email and a notification in Folia that opens the new contact. If Folia holds one for you to review, the email says so and opens it on the Submissions tab. You can turn these off in Settings → Notifications, under Inquiries.
- To see new inquiries, open Contacts in the CRM. You can also open the form and click the Submissions tab to read everything sent through it, including answers that weren't saved to the contact.
- A few submissions may wait for you. If one looks like it was sent by a computer program rather than a person, Folia holds it in To review instead of adding a contact. On the Submissions tab, read it, then click Accept to add the couple as a contact, or Discard if it's spam. The To review column on the forms list shows how many are waiting.
When a couple books, open their contact and choose Create wedding. Their names, email, phone, date, guest count and budget are filled in for you. If you've already added their wedding, choose Link existing wedding instead.
Changing a live form
You can edit a form after it's live. On a live form, the Save button is called Update live form, and your changes reach couples as soon as you click it.
Your link and the code on your website stay the same, so there's nothing to update anywhere else.
Taking a form down
- Unpublish turns a live form off. The button is at the top right of the form. Your link and your website show "not available" until you publish again, and everything couples already sent is kept.
- Archive puts away a form you no longer need. Click Archive form at the bottom of the Settings tab, or click ⋯ next to the form on the forms list, then Archive. The link stops working, and the submissions and contacts it brought in are kept.
To bring back an archived form, click Archived at the top of the forms list, click ⋯ next to the form, then Restore. It comes back as a draft, ready to publish again.
Worth knowing
- Send yourself a test. Fill out your form once from your phone or computer, then look for yourself in Contacts. To remove the test contact, click ⋯ next to it, then Delete.
- Each submission is a new contact. If the same couple sends your form twice, they appear in Contacts twice.
- "This form isn't available" means the form is a draft, unpublished, or archived. Publish it, and the same link works again.
- Forms on your directory profile. A form with a Directory profile badge is the one couples use to contact you from your Folia directory profile. Your profile always has one: Folia gives it a standard form with the usual questions, and you can edit those questions or switch to another published form. It can't be unpublished or archived while your profile uses it. To switch forms, go to your profile's Inquiries section.
